Dietary Services

OUR Dietary Department prepares a variety of different diets to meet the needs of our residents in accordance with their physician’s recommendation. Meals are planned not only to meet one’s nutritious needs, but also to take into consideration one’s individual preferences, as well as their ethnic and religious backgrounds.

Our residents are encouraged to eat in our dining room. We have a special feeding program for those requiring some assistance in this area.

Evergreen Provides a Variety of Services:

Short term rehabilitation services including:
  • Physical, occupation, speech and respiratory therapies
  • Hydro therapy
  • Intravenous therapy
  • Nasogastric or gastrostomy feeding
  • Colostomy care
  • Wound and skin care
  • Long term care for persons unable to live independently and requiring skilled nursing services
  • Dental, podiatry and ophthalmology consultation and care
  • Pharmaceutical services

Activities

The residents at Evergreen are encouraged to be as independent and active as possible. The Activity staff fosters this independence by providing activities that are meaningful, fun and serve to build one’s self esteem and morale.

Daily activities include:

  • Musical entertainment
  • Games
  • Arts & crafts
  • Ceramics
  • Religious programs
  • Parties
  • Movies
  • Travel programs
  • Cooking
  • Outings
  • Exercise and fitness
  • In-room programs for those unable to participate in group activities
  • Intergenerational activities such as the Adopt-a-Grandparent program
Family members are encouraged to participate in all these programs with our residents.

Resident Council

Through participation in the resident council, residents and family members have a voice in what goes on within the facility. The council provides an opportunity for them to express their suggestions and concerns, so that the administration and staff can serve their needs.

Social Services

Our Director of Social Services works with residents and families to assist them in adjusting to placement in a skilled nursing facility. The director is a good listener and acts as a liaison between residents, family members and the hospital staff. Upon discharge, the Social Service Director can assist with making plans for the in-home care and will follow-up to make sure that the transition to home goes smoothly.
