Daniel J. Cipponeri, FACHA Administrator of Evergreen for over 25 years extends to you a cordial welcome to the facility. He has an “open door” policy and is available to meet with family members and residents upon request.

In addition, Evergreen has a dedicated management team comprised of years of experience and knowledge in quality patient services.

Our Professional Staff

Here at Evergreen our goal is to provide quality care to our residents. Nursing is provided by licensed, registered and vocational nurses and certified nurse aids, as directed by the resident’s physician. All nursing staff receives ongoing education and in-service instruction to keep them abreast of new and better techniques in order to provide continuing quality care. Evergreen takes pride in low employee turnover and each and every one works With a Smile and a Warm Heart.

Evergreen Nursing & Rehabilitation Care Center is licensed by the State of California to provide 24 hour skilled nursing care to the chronically ill and disabled. Our facility is certified by Medicare and Medi-Cal.

The Board of Quality Assurance/Advisory Committee

Our Medical Director, Medical Consultant, Quality Assurance Consultant, Administrator, staff consultants and the department directors meet frequently to review, revise and develop new policies and procedures related to resident care. They routinely review the activities of each department and are always looking for better and more efficient ways to meet the needs of our residents.
